Services Provided: The Illinois Department on Aging provides a
statewide, toll-free, telephone "Information and Assistance" service
for persons 60 years of age and older and their caregivers. Services include
Abuse, Neglect and Exploitation intake; legal services; Nursing Home Ombudsman
program; Case Management Services; nutrition and social services; Information
and Assistance on public benefit programs; private pay services; alternative
care to nursing home placement, and community based providers. Employment
assistance is available to individuals 55 and older through Employment
Specialists statewide.
Alternative
care is provided through the Department’s "Choices for Care" program
which provides a comprehensive assessment to advise
seniors of various options. The "Community Care Program" provides
Homemaker and Adult Day Care services. Respite care is available and Senior
Companion in some parts of the state. Individuals and couples can utilize the
services of the "Community Care Program". For couples, the
"Spousal Impoverishment Standard" has provided an opportunity for individuals
to receive services who were not previously eligible because of assets.
Information and literature describing these programs are available, free of
charge, through the Senior Helpline toll free number.
Eligibility: 60 years or older, need in-home care, meet asset requirements;
an assessment for determination of need will establish level and extent of

Security Income), Qualified Medicare Beneficiary Program, etc. Outreach to the homebound to help them receive
benefits they may be entitled to. Assessment
of the need for, and development of, a Care Plan for those who need help with
laundry, cooking, housekeeping, personal care, etc. (Some qualify for payment
of this service through a state-supported program.) Also do "Assessment of
Need" for the Salvation Army Home-Delivered Meal program. Pre-screening for nursing home admissions and investigation of
Elder Abuse allegations. Counseling for those who may
be experiencing difficulty dealing with the problems and ailments of aging.
Assistance with finding a long-term care facility, and serve
as an advocate for the rights of the residents. Transportation to
grocery shopping, malls, personal and/or medical
appointments (wheelchair-lift van); must be scheduled ahead of time and is
subject to availability. Generally, medical appointments are made on Tuesdays
and Thursdays. Senior Health Insurance Program (SHIP) volunteers help to sort
through Medicare bills and explanation of Medicare Benefits forms, and give
information on various Medicare Supplemental Insurance programs.
Eligibility: Must
be 60 years or older. Fee: A suggested donation for
transportation to and from lunch or to and from doctor's office in
S.H.A.R.E.
